Payment Policy

Payment is due at the time of appointment booking to reserve your appointment on the Treasured Labs LLC schedule. Your appointment time is held exclusively for you, and payment confirms and secures your reserved time slot.

 

 

Cancellation Policy

Appointments must be canceled or rescheduled no later than 6:00 PM (Arizona time) on the day before your scheduled appointment.

Late cancellations, missed appointments, or patients who are unavailable at the scheduled appointment time are not eligible for a refund.

No-Refund / Unsuccessful Blood Collection Policy

Every reasonable effort will be made to successfully obtain the requested specimen. Payment covers your reserved appointment time, travel, medical supplies, and professional mobile phlebotomy services. It does not guarantee successful specimen collection.

If Treasured Labs LLC is unable to obtain a specimen due to difficult veins, dehydration, patient movement, refusal, safety concerns, or other circumstances beyond our control, no refund will be issued.

Laboratory Responsibility Disclaimer

Treasured Labs LLC provides professional specimen collection and transportation services only.

Treasured Labs LLC is not responsible for laboratory testing, specimen processing, reporting of results, laboratory billing, delays, lost specimens after transfer, or interpretation of laboratory results.

This applies to, but is not limited to:

• Sonora Quest Laboratories

• Labcorp

• Patient Choice Laboratories

• Hospital laboratories

• Specialty laboratories

• Reference laboratories

• Any laboratory selected by the ordering healthcare provider or patient

Questions regarding laboratory testing, processing, billing, or results should be directed to the performing laboratory or your ordering healthcare provider.

Safe Environment & Professional Conduct Policy

Treasured Labs LLC is committed to providing a safe and respectful environment for every patient, family member, and healthcare professional.

For the safety of our patients and staff, Treasured Labs LLC reserves the right to postpone, discontinue, or end an appointment if the environment becomes unsafe, threatening, disruptive, or if anyone present behaves in a manner that places the patient or provider at risk.

Examples include, but are not limited to:

• Aggressive or threatening behavior

• Unsafe home conditions

• Loose or aggressive pets

• Intoxication or impairment

• Unsafe sanitary conditions

• Any situation that prevents safe patient care

 

Payment remains non-refundable if an appointment cannot be completed due to unsafe conditions.

Preparation Before Your Appointment

To help ensure a smooth appointment, please have the following available before your scheduled visit:

• Laboratory order

• Photo identification

• Insurance information (if applicable)

• HSA/FSA card (if applicable)

• A well-hydrated patient unless otherwise instructed by the ordering provider

• A quiet, well-lit area for the blood draw

 

Please notify Treasured Labs LLC before your appointment if the patient has a history of fainting, difficult blood draws, bleeding disorders, or special accommodations that may help us provide the best care.

After Your Blood Draw

After your blood draw:

• Keep the bandage in place for at least 15–30 minutes.

• Avoid strenuous activity or heavy lifting with the affected arm for several hours.

• Drink plenty of fluids unless otherwise instructed by your healthcare provider.

• Mild bruising or tenderness at the collection site is common and usually resolves within a few days.

 

If you experience excessive bleeding, severe pain, swelling, signs of infection, or any unusual symptoms, contact your healthcare provider immediately.

 

 

Privacy & Confidentiality

Treasured Labs LLC is committed to protecting your privacy. Patient information is handled in accordance with applicable privacy laws and professional standards.

Information collected during your appointment is used only for providing mobile phlebotomy services and communicating with you regarding your care.

Patient Acknowledgement

By scheduling an appointment with Treasured Labs LLC, I acknowledge that I have read, understand, and agree to the Payment Policy, Cancellation Policy, No-Refund/Unsuccessful Blood Collection Policy, Laboratory Responsibility Disclaimer, Safe Environment Policy, and all other policies listed on this page.

I understand that payment reserves my appointment time and is not contingent upon successful specimen collection or laboratory testing.
