If you do not agree to these terms, please do not use our services. 2. Description of Services Treasured Labs provides mobile phlebotomy (blood drawing) and specimen collection services. We are a collection service only. - We do not provide medical advice, diagnose conditions, or treat illnesses. - We do not run the laboratory tests; we only collect and transport the specimens to the designated, certified laboratory. - You must consult your physician regarding your test results. 3. User Responsibilities To ensure a safe and successful mobile visit, you agree to: - Provide accurate, complete, and up-to-date personal, medical, and payment information. - Have a valid doctor’s order or test requisition ready at the time of your appointment. - Provide a safe, clean, and well-lit environment for the phlebotomist to perform the draw. - Follow any pre-draw instructions provided by your physician (e.g., fasting, drinking water). - Disclose any history of fainting, severe bleeding, or complications with past blood draws to the phlebotomist prior to the procedure. 4. Appointments, Cancellations, and Delays - Arrival Windows: Because we are a mobile service, appointment times are estimated windows. We will communicate any significant delays caused by traffic or weather. - Cancellations: We require at least [Insert Number, e.g., 24] hours' notice for cancellations.
